What Do Spaying and Neutering Involve?

Spaying a Female Dog or Cat

Spaying is performed under general anesthesia. The procedure removes the ovaries and, depending on the surgical technique and patient, may also remove the uterus.

Removing the ovaries stops heat cycles and prevents pregnancy. When the reproductive tract is removed or hormonally inactivated through surgery, the patient can no longer develop pyometra associated with an intact reproductive system.

A spay is abdominal surgery and generally requires a longer recovery period than a routine male neuter.

Neutering a Male Dog or Cat

Neutering, also called castration, removes both testicles under general anesthesia.

The procedure prevents the male from producing sperm and substantially reduces testosterone production. The surgery is usually performed through a small incision, although technique and recovery depend on the patient.

Cryptorchid Neuter Surgery

A cryptorchid male has one or both testicles retained in the abdomen or inguinal area rather than normally descended into the scrotum.

Retained testicles should generally be removed because they have an increased risk of torsion and testicular tumor development. Cryptorchid surgery is more involved than a routine neuter because the veterinarian must locate and remove the retained testicle.

A cryptorchid animal should not be bred because the condition has a hereditary component.

Potential Benefits of Spaying or Neutering

Prevention of Unplanned Pregnancy

Spaying and neutering prevent your pet from producing an accidental litter. Even owners who intend to be responsible can encounter an unexpected mating when a gate is left open, a dog escapes during a heat cycle or an indoor cat slips outside.

Prevention of Pyometra

Pyometra is a potentially life-threatening uterine infection that develops in intact female dogs and cats. It may cause lethargy, reduced appetite, vomiting, increased thirst and urination, vaginal discharge, abdominal enlargement, sepsis or collapse.

Spaying prevents future pyometra when the ovaries and reproductive tissue responsible for the condition have been removed.

Elimination of Testicular Cancer

Removing both testicles eliminates the possibility of developing testicular cancer later in life.

Reduction of Some Prostate and Hormone-Related Conditions

Neutering greatly reduces the likelihood of benign prostatic enlargement and can reduce the risk of some testosterone-dependent conditions, including certain perianal tumors.

Neutering does not prevent every prostate disorder. Neutered males can still develop urinary disease, infection and, less commonly, prostate cancer.

Reduced Desire to Roam in Some Males

As intact males reach sexual maturity, they may become strongly motivated to leave the home or yard when they detect a female in heat.

Neutering reduces the breeding drive and can make roaming, escape attempts, mounting and urine marking less likely. It does not guarantee that these behaviors will disappear, especially when they have become learned habits or are caused by anxiety, territorial behavior or insufficient enrichment.

The American Veterinary Medical Association’s spay and neuter resource discusses reduced roaming and urine marking among the potential behavioral effects of neutering.

Elimination of Heat Cycles

Spaying prevents future heat cycles, including:

  • Vaginal bleeding or discharge
  • Attraction of intact males
  • Restlessness or vocalization
  • Escape attempts
  • False pregnancy in some dogs
  • The need to prevent mating throughout the fertile period

Possible Reduction in Mammary-Tumor Risk

Spaying before repeated heat-cycle exposure may reduce the risk of mammary tumors in female dogs. However, the strength of the evidence and the magnitude of that protection are less certain than many older educational materials suggest.

This issue is discussed in greater detail below because mammary-tumor prevention must be balanced with orthopedic, urinary and other breed-specific considerations.

At What Age Should My Pet Be Spayed or Neutered?

There is no single age that is ideal for every patient.

The optimal timing may differ substantially between:

  • A five-pound indoor cat
  • A small-breed dog
  • A Golden Retriever
  • A giant-breed dog
  • A dog with a high risk of accidental breeding
  • A performance or working dog
  • A patient with an orthopedic condition
  • A female whose family cannot safely manage a heat cycle

Spay and Neuter Timing for Cats

Cats can become reproductively mature at a young age. Female cats may enter their first heat and become pregnant while they still appear kitten-like, and males may begin roaming, spraying and breeding.

For most healthy pet cats not intended for breeding, sterilization is commonly recommended by approximately five months of age and before the first heat or opportunity to reproduce.

The appropriate date still depends on:

  • Health and body condition
  • Vaccination and infectious-disease status
  • Whether the cat is indoor-only or may escape outdoors
  • Whether male and female kittens live together
  • Any congenital or developmental concern
  • The veterinarian’s examination

The AVMA provides professional resources supporting the veterinary consensus that cats not intended for breeding should generally be sterilized by five months of age.

Small- and Medium-Breed Dogs

For many smaller dogs, surgery is commonly considered around five to six months of age.

For a female dog, an owner may choose surgery before the first heat when priorities include:

  • Avoiding accidental pregnancy
  • Avoiding management of a heat cycle
  • Preventing pyometra as early as possible
  • Potentially reducing lifetime mammary-tumor risk

For a male small-breed dog, approximately six months is a common starting point, provided both testicles are descended and the patient is healthy.

Large- and Giant-Breed Dogs

Large and giant dogs mature more slowly. In some breeds, sterilization before skeletal maturity has been associated with increased risks of selected orthopedic disorders and, in some studies, certain cancers.

For many dogs projected to weigh at least 45 pounds as adults, AAHA suggests discussing surgery after growth is substantially complete—often between approximately 9 and 15 months. Some giant breeds or breed-specific situations may justify a longer delay.

Female timing can be more complicated because the first heat may occur before complete skeletal maturity. The decision may require balancing:

  • Orthopedic development
  • Breed-associated cancer data
  • Risk of accidental pregnancy
  • Ability to safely manage heat cycles
  • Pyometra risk
  • Mammary-tumor considerations
  • Urinary-incontinence considerations
  • The pet’s lifestyle and home environment

Breed-Specific Timing

Research from the University of California, Davis has found that associations between sterilization age, joint disease and cancer vary significantly among dog breeds.

This does not mean every dog of a particular breed will develop a problem. It means that breed, sex and adult size may provide additional information when deciding whether to operate before puberty, around puberty or after growth.

Owners of large, giant, athletic and working breeds may review the UC Davis breed-specific spay and neuter resource and discuss how the findings apply to their individual dog.

Potential Benefits and Risks of Waiting

Possible Benefits of Waiting in Selected Dogs

Allowing additional time for natural sex-hormone exposure and skeletal maturation may be considered in selected larger dogs.

Potential reasons to wait may include:

  • Reducing the risk of certain joint disorders in susceptible breeds
  • Allowing growth plates more time to close
  • Breed-specific evidence involving certain cancers
  • Reducing the risk of urethral sphincter mechanism incontinence in selected females
  • Supporting development in certain working or performance dogs

These considerations are most relevant to dogs. They do not establish that delaying surgery is beneficial for every breed or every individual.

Risks and Responsibilities of Waiting

Delaying surgery means the pet remains fertile and continues to be exposed to reproductive hormones.

Owners must be prepared to manage:

  • Accidental-pregnancy risk
  • Heat cycles in females
  • Attraction of intact males
  • Roaming or escape attempts
  • Urine marking or mounting
  • Possible dog-to-dog conflict
  • Pyometra risk in females
  • Testicular and prostate disease in males
  • Potential mammary-tumor risk with additional heat cycles
  • Boarding, daycare or housing restrictions involving intact pets

A female in heat must be kept completely separated from intact males throughout the fertile period. A fence, diaper or brief supervision is not reliable pregnancy prevention.

Waiting Is Not the Same as Choosing Not to Spay or Neuter

Some families decide to delay surgery until a dog reaches a particular stage of development and then proceed. That is different from leaving a pet intact indefinitely without a reproductive-management plan.

We recommend deciding in advance:

  • The anticipated target age
  • How heat cycles will be managed
  • How accidental breeding will be prevented
  • What signs of pyometra require urgent care
  • Whether follow-up growth or orthopedic assessment is needed

Spaying and Mammary Cancer: What Does the Evidence Show?

Mammary tumors are among the most important reproductive cancers in intact female dogs. Exposure to ovarian hormones is biologically relevant to mammary-tissue development, and many studies have reported fewer mammary tumors among dogs spayed earlier in life.

The Frequently Quoted Historical Numbers

Many veterinary websites state that a female dog’s mammary-tumor risk is:

  • Approximately 0.5% of an intact dog’s risk when spayed before the first heat
  • Approximately 8% after one heat
  • Approximately 26% after two heats

These figures originated primarily from a study published in 1969. They should not be presented as precise modern lifetime probabilities for every dog.

Why the Evidence Requires Caution

A 2025 systematic review evaluated 13 studies examining spay timing and canine mammary-tumor development. Seven suggested a protective effect from earlier spaying, while six did not find evidence of that protection.

The reviewers concluded that:

  • Earlier spaying probably provides some mammary-tumor protection
  • The magnitude of protection remains uncertain
  • Many studies were retrospective and vulnerable to bias
  • Breed, age, body condition, diet and other factors were not consistently controlled
  • Mammary-tumor prevention should be balanced against other health considerations

How We Use This Information

We discuss mammary-tumor risk as one legitimate consideration—but not the sole reason to perform every dog’s surgery at the same age.

For a small female dog with low orthopedic risk and significant accidental-pregnancy risk, spaying before the first heat may be a reasonable choice.

For a large or giant female with breed-associated joint concerns, the family may reasonably discuss waiting until additional skeletal maturity while accepting and carefully managing the reproductive risks.

The decision should reflect the patient and family rather than repeating one statistic without context.

Spaying, Neutering and Pet Overpopulation

Preventing an unplanned litter is one of the clearest benefits of spaying and neutering.

One accidental litter may produce several puppies or kittens, and those offspring can reproduce within months. Even when every animal in a litter initially finds a home, those placements compete with dogs and cats already waiting in shelters and rescues.

Best Friends Animal Society estimates that in 2025:

  • Approximately 4.74 million dogs and cats entered U.S. shelters
  • Approximately 209,000 dogs were killed in shelters
  • Approximately 187,000 cats were killed in shelters
  • Approximately 396,000 dogs and cats were killed in shelters overall

National shelter organizations use different reporting methods and definitions, but the central point is unchanged: shelters and rescue organizations continue to receive more animals than available homes and resources can accommodate in many communities.

Spaying and neutering are not the only solutions to shelter crowding. Adoption, fostering, microchipping, pet-friendly housing, access to veterinary care and keeping pets with their families also save lives. Preventing accidental litters remains an important part of that broader effort.

Retained Deciduous Teeth at the Time of Spay or Neuter

Puppies and kittens begin with deciduous, or baby, teeth that should fall out as their permanent teeth erupt.

A retained deciduous tooth remains in place after the corresponding adult tooth begins to erupt. This is especially common in small and toy-breed dogs and often affects the canine teeth.

Retained baby teeth can contribute to:

  • Crowding
  • Food and plaque accumulation
  • Early periodontal disease
  • Displacement of the adult tooth
  • Abnormal bite alignment
  • Trauma to the palate or other oral tissues
  • Pain or infection

Can Retained Baby Teeth Be Removed During Spay or Neuter Surgery?

When a retained deciduous tooth is identified and removal is appropriate, it may sometimes be extracted during the same anesthetic event as a spay or neuter.

Combining procedures can avoid a second anesthetic event, but the decision depends on:

  • Which tooth is retained
  • Whether the permanent tooth is present and erupting
  • Root anatomy
  • Whether dental radiographs are needed
  • The anticipated length of the combined procedures
  • The pet’s anesthetic status

Retained deciduous canine teeth have long, delicate roots and must be extracted carefully to avoid root fracture or injury to the developing permanent tooth.

Do Not Always Wait Until Spay or Neuter

If a permanent tooth is already erupting beside a retained baby tooth, earlier extraction may be recommended. Waiting several additional months solely to combine it with a later spay or neuter may allow permanent malocclusion or crowding to develop.

Pre-Anesthetic Laboratory Testing

Every spay or neuter patient receives a preoperative evaluation. Blood work provides information that cannot be obtained through a physical examination alone.

Pre-anesthetic testing may include:

  • A complete blood count
  • A blood-chemistry profile
  • Blood glucose
  • Kidney and liver indicators
  • Electrolytes
  • Protein levels
  • Additional testing based on age, breed, history or examination findings

What Can Pre-Anesthetic Blood Work Identify?

Testing may reveal:

  • Anemia
  • Inflammation or infection
  • Low platelet numbers
  • Kidney or liver abnormalities
  • Blood-glucose abnormalities
  • Electrolyte changes
  • Dehydration
  • Low protein levels
  • Other findings that could affect anesthesia or recovery

A normal laboratory panel cannot guarantee that an anesthetic complication will not occur. It helps the veterinarian identify known risk factors, select medications, plan fluid therapy and decide whether surgery should proceed, be modified or be postponed.

Spaying and neutering are common procedures, but they are still surgeries performed under general anesthesia. “Routine” should never mean casual.

Can Other Services Be Completed During the Same Anesthetic Event?

Depending on your pet’s needs and the expected anesthetic time, we may discuss coordinating selected procedures such as:

  • Extraction of retained deciduous teeth

  • Microchip implantation
  • Selected minor procedures

Combining procedures is not always appropriate. The veterinarian considers the total anesthetic time, pain, cleanliness of each procedure, recovery needs and whether delaying treatment would cause harm.

Recovery After Spay or Neuter Surgery

Most patients go home the same day after they are awake, stable and able to recover safely outside the hospital.

Follow the discharge instructions provided for your individual pet.

General Home-Care Instructions

  • Keep your pet indoors, quiet and supervised.
  • Prevent running, jumping, rough play and off-leash activity.
  • Use the prescribed recovery collar or surgical garment continuously.
  • Check the incision at least once or twice daily.
  • Keep the incision clean and dry.
  • Do not bathe or allow swimming until the veterinarian approves it.
  • Give medication exactly as directed.
  • Do not give human pain medication.
  • Attend any recommended recheck appointment.

How Long Is Activity Restricted?

Many pets require approximately 10–14 days of activity restriction, although the exact period depends on the procedure, incision, patient and healing progress.

Male cats often feel active quickly, but the surgical site still needs protection. Feeling energetic does not mean internal tissues have fully healed.

  • Difficulty breathing

Frequently Asked Questions About Spaying and Neutering

Should every dog be spayed or neutered at six months?

No. Six months remains appropriate for many cats and smaller dogs, but larger dogs may benefit from a later, breed-specific discussion. The decision should consider size, sex, growth, health, behavior and accidental-breeding risk.

Should a female dog have one heat or one litter first?

There is no general medical need for a female dog to have a litter before being spayed. Allowing a heat cycle may be part of an individualized timing decision for some larger dogs, but it creates pregnancy risk and additional reproductive-hormone exposure.

Will neutering stop aggression?

Not necessarily. Neutering may reduce some testosterone-related behaviors, particularly roaming, mounting, marking and conflict between intact males. Fear, anxiety, pain, resource guarding and learned aggression require separate evaluation and treatment.

Will neutering stop my male dog from running away?

It may reduce sexually motivated roaming and escape attempts. It will not eliminate roaming caused by boredom, separation anxiety, inadequate fencing, prey drive or learned behavior.

Will my pet gain weight after surgery?

Calorie needs may decrease after sterilization, but weight gain is not inevitable. Portion control, appropriate nutrition, exercise and regular weight monitoring can keep most pets at a healthy body condition.

Can my dog be spayed while she is in heat?

It may be possible, but the reproductive tissues have increased blood supply during heat, which can make surgery more complicated. The veterinarian will discuss timing, urgency and individual risks.

Can my pet be spayed or neutered if older?

Yes, many adult and senior pets can undergo surgery safely after appropriate evaluation. Age alone is not a disease, but older patients may need more extensive laboratory testing or additional diagnostic assessment.

What if only one testicle has descended?

The retained testicle must also be located and removed. A cryptorchid neuter is more involved than a routine neuter and may require abdominal or inguinal exploration.

Can retained baby teeth be removed during surgery?

Often, yes, when the tooth requires extraction and combining procedures is medically appropriate. However, a retained tooth should sometimes be removed earlier rather than waiting for a delayed spay or neuter date.

Does my pet need blood work before surgery?

Pre-anesthetic laboratory testing is recommended according to the individual patient’s age, health and anesthetic risk. It can identify abnormalities not apparent on the physical examination.

Do you give pain medication?

Yes. Spay and neuter patients receive an individualized pain-management plan before, during and after surgery.

How long will my pet wear a cone?

Most pets need incision protection throughout the healing period, commonly approximately 10–14 days. Follow the specific instructions provided at discharge.

How much does spay or neuter surgery cost?
